Individual membership fee of $125/month for 12 months.
REFUND AND CANCELLATION POLICY
- If a member cancels his/her membership within 30 days from the start of the membership and prior to receiving dental treatment or consult by participating dentist, the member will receive a full refund of sum paid.
- If a member cancels his/her membership after 30 days from the start of the membership and prior to receiving dental treatment or consult by participating dentist, the member will not be held responsible for the remainder of the monthly payments left in the 12 months contract .
- If a member cancels or defaults on monthly membership payments after receiving dental treatment by participation dentist(s), the member will be responsible for a full cost of received dental treatment based on non-membership Fee Schedule. The member’s monthly payments and lab fees (if any) made prior to cancellation or default will be taken into account.
- The above services are provided by participating General Dentists at participating locations only.
Services of Dental Specialists (endodontists, periodontists, oral surgeons, orthodontists and etc...) will not be provided under DENTbenefits Membership Program.
- If dental procedure (example: root canal treatment, extraction) is initiated by a participating general dentist but it is later determined by the participating general dentist that it needs to be referred to a dental specialist, the dental procedure will not be covered under the plan. The patient will be responsible for the cost of treatment by the dental specialist.
UNLESS YOU NOTIFY US BEFORE A CHARGE THAT YOU DO NO WANT TO AUTO RENEW, YOU UNDERSTAND YOUR MEMBERSHIP WILL AUTOMATICALLY CONTINUE AND YOU AUTHORIZE US (WITHOUT NOTICE TO YOU, UNLESS REQUIRED BY APPLICABLE LAW) TO COLLECT THE THEN-APPLICABLE MEMBERSHIP FEE, USING ANY CREDIT CARD WE HAVE ON RECORD FOR YOU.
DentBenefits Dental membership program is accepted only by participating dentist(s) at participating location(s) only.
